Responsive client is seeking to change the name on her birth certificate so it matches the name used on her other forms of identification. Client’s birth certificate currently features a misspelled version of her first name, as well as her maiden name. Due to the discrepancy between her birth certificate and other forms of identification, client is unable to get a REAL ID and cannot travel as a result. VIP has a copy of client’s fingerprints and birth certificate. A volunteer is needed to file a name change petition on behalf of the client and assist with amending her birth certificate.
